Thank you for participating in the "$100,000 Giveaway.” There is no purchase necessary. Simply complete this form and submit at point of promotion. Multiple entries and unreadable entry forms will be disqualified. You must be at least eighteen years old in order to be awarded any prizes. Winners of the $100,000 Giveaway will have a choice of their Dream Prize/Car worth up to $100,000 or $100,000 Cash.
All entries will be automatically entered for a chance to win the "Choose Your Prize" Giveaway and the "Weekly Prize" Giveaway.
Winners of the Choose Your Prize Giveaway will select one of several prizes each valued at $5000. Examples of these prizes, which may be on display at this location, include: $5000 in Free Gas Cards, an Off-Road Vehicle valued at $5000, Free Movie Gift Cards valued up to $5000, other prizes valued up to $5000, or $5000 cash. Any Choose Your Prize gifts that may be displayed are an example only and total value of any awarded gifts shall not exceed $5000. The drawing for the Choose Your Prize Giveaway winner will be held December 10, 2008 at the corporate office of Pacific Monarch Resorts, Inc. (PMR).
At least one Weekly Prize will be awarded each week from all entry forms received as of each Sunday at 6:00 P.M. 52 or more Weekly Prize prizes will be awarded each week throughout 2008. Weekly Prizes consist of various gifts such as retailer gift cards, and vacation certificates for Carnival Cruises, Las Vegas, Hawaii, Palm Springs, Big Bear and many other top vacation destinations. PMR reserves the right to add or remove prizes from the list of Weekly Prizes.
Winners under the Choose Your Prize and Weekly Prize Giveaways will be determined by a random drawing from all entries received as of the drawing. Accordingly, the odds of winning a prize during the Choose Your Prize Giveaway drawing or during any particular Weekly Prize drawing depend upon the number of entries received for that drawing.
Awarded gifts or prizes may not be exchanged for cash. A complete list of Choose Your Prize and Weekly Prize winners and their prizes will be posted no later than 30 days following each drawing at www.entertowin.net/winners. PMR reserves the right to offer a rain check or prize of equal value for any prize not immediately available.
The $100,000 Giveaway, the Choose Your Prize Giveaway, and the Weekly Prize Giveaway are sponsored and administered by PMR located at 23091 Mill Creek Drive, Laguna Hills, CA 92653 whose business is vacation timeshare resort ownership.
Participants who meet the age and income requirements under the $100,000 Giveaway may be contacted by PMR via telephone, mail or email. By signing and submitting this entry form, you are granting PMR express permission to contact you at the telephone number(s) you have provided on this form even if such telephone number(s) have been registered on the national do not call list.
All gifts and prizes associated with the $100,000 Giveaway may only be delivered following attendance at a PMR sales presentation, and cannot be awarded via the telephone, email, the Internet or regular mail. Acceptance of any gift or prize constitutes your permission for PMR's use of your name and likenesses for promotional purposes without further compensation except where prohibited by law.
The $100,000 Giveaway is open only to eligible participants, including married couples and single homeowners 25 to 75 years of age with an annual income of at least $35,000. Submitted entry forms are the property of PMR and may be used for secondary promotional purposes. Current and previous employees of PMR and/or their affiliated entities and their families are not eligible to participate in this promotion. This giveaway is not promoted, sponsored and/or administrated by the management or owner of any point of promotion location, including American Multi-Cinemas, Cinemark Inc., Regal Entertainment Group, Fandango, any of their affiliates or by any manufacturer of any gift.
